一天注射一次的 insulin glargine 需 2-4 天才會達到穩定期
(Toujeo 需五六天)

[儲存]
儲存於 2℃至 8℃（冰箱中冷藏）。
存放在外包裝紙盒內。 請勿冷凍。
注射匣請勿置入冷凍室或冷凍盒中。
第一次使用前，注射筆需先存放在室溫下 1 到 2 小時。
使用期間請勿儲存在高於 25℃的環境。
開封後的注射筆不要放入冰箱。

Insulin glargine injected once daily will reach steady state levels in 2-4 days after the first dose.

從其他胰島素轉成使用 lantus  (Switch from other insulins to Lantus )

原本使用中效或長效insulin, 轉換成使用 lantus, 基礎胰島素劑量可能需調整, 其他抗糖尿病藥物也可能需要調整., 

When switching from a treatment regimen with an intermediate or long-acting insulin to a regimen with Lantus, a change of the dose of the basal insulin may be required and the concomitant antidiabetic treatment may need to be adjusted (dose and timing of additional regular insulins or fast-acting insulin analogues or the dose of oral antidiabetic medicinal products). 

從一天注射兩次的 NPH 換成 LANTUS, 為了避免夜間低血糖或清晨低血糖, 建議前幾周使用時, 將每日總量減低 20-30%, 

Switch from twice daily NPH insulin to Lantus 

To reduce the risk of nocturnal and early morning hypoglycaemia, patients who are changing their basal insulin regimen from a twice daily NPH insulin to a once daily regimen with Lantus should reduce their daily dose of basal insulin by 20-30% during the first weeks of treatment. 

從 Gla-300 換成 LANTUS 時, LANTUS 和 TOUJEO 兩者生體效果並不相等, 因此不能相同劑量直接轉換, 如果從 GLA-300 換成 LANTUS, 建議減量 20%, 最初幾周, 補充隨餐胰島素來代償減量的效果, 之後依照病患狀況個別做調整, 轉換藥物期間要頻繁偵測血糖, 

Switch from insulin glargine 300 units/ml to Lantus

Lantus and Toujeo (insulin glargine 300 units/ml) are not bioequivalent and are not directly interchangeable. To reduce the risk of hypoglycemia, patients who are changing their basal insulin regimen from an insulin regimen with once daily insulin glargine 300 units/ml to a once daily regimen with Lantus should reduce their dose by approximately 20%. During the first weeks the reduction should, at least partially, be compensated by an increase in mealtime insulin, after this period the regimen should be adjusted individually. Close metabolic monitoring is recommended during the switch and in the initial weeks thereafter.

[劑量及用法]
Lantus 可使用於成人，青少年及 6 歲以上（含 6 歲）之糖尿病患。
Lantus 含作用時間較長的胰島素類似物 insulin glargine，故可於一天中任選某一時間，但固定於每
天同一時間注射一次。
Lantus 單次使用注射筆（OptiSet）每次轉動刻度之增加量為 2IU，單次最大劑量為 40IU。
應依病患個別狀況調整 insulin glargine 劑量。對於第 2 型糖尿病患，可以與口服抗糖尿病藥併用。
